Host Home Program:
The Host Home Program provides individuals with intellectual
disabilities a safe, supportive, and welcoming place to live. Each
individual shares a home with a caring person or family who
provides consistent support and includes them in everyday
household life. Over time, the individual becomes a valued
member of the home, building meaningful relationships and
enjoying greater stability without the disruption of frequently
changing staff.
What Is a Host Home?
A Host Home is a private, community-based home in which the
Host lives and provides daily support as an independent
contractor of Solace Hill LLC. It is not a shift-staffed group home or simply a room rental. The arrangement is built around
compatibility, trust, safety, privacy, and shared community life. A home may support one individual or, when assessed,
authorized, and appropriately matched, no more than two
unrelated individuals.
Who May Benefit?
• Adults with intellectual disabilities authorized for Iowa
Medicaid Supported Community Living services.
• People who prefer a stable, relationship-based home rather
than rotating staff.
• Individuals who want greater independence, meaningful
relationships, personal growth, and community involvement.
• People who can be safely supported in this model and
choose it through person-centered planning.

- Medication Administration: Assisting with and managing daily medication schedules.
- Individualized Support: Delivering tailored daily care based on personal needs.
- Interdisciplinary Collaboration: Participating in team meetings to review and optimize individual care.
