Supported Community Living

Solace Hill LLC provides Daily Supported Community Living services for adults with intellectual disabilities who need consistent assistance and supervision to live safely and successfully in the community. Services are provided in comfortable, provider-operated homes located in ordinary residential neighborhoods. Depending on each individual’s assessed needs and authorized person-centered service plan, support may be available 24 hours a day, seven days a week.

Our trained staff provide individualized assistance while promoting independence, dignity, personal choice, and community involvement. Services may include:

Person-Centered Community Living

Each individual participates in decisions about their home, daily schedule, personal goals, relationships, and community activities. Solace Hill works closely with the individual, guardian when applicable, case manager, managed care organization, healthcare providers, and interdisciplinary team. Our homes promote:

Who May Benefit?

Daily Supported Community Living may be appropriate for an adult who:

  • Has an intellectual disability
  • Is authorized for Iowa Medicaid Supported Community Living
  • Needs regular, overnight, or 24-hour support
  • Benefits from structure, supervision, and consistent staff assistance

Chooses a provider-operated home through person-centered planning
